NHS Review Finds 58% of Serious Birth Tears Misdiagnosed

A new NHS Resolution review has shed light on a deeply concerning pattern in maternity care.

Serious perineal tears during childbirth are being missed or misdiagnosed far too often and for women living with the consequences, the impact can be life?changing not just physically but also, emotionally.

The thematic review by NHS Resolution in collaboration with the Royal College of Obstetricians and Gynaecologists analysed 237 obstetric anal sphincter injury (OASI) clinical negligence claims over a ten?year period and found that 58% of women had their tear misdiagnosed, often graded as a minor second?degree tear when it was actually a third? or fourth?degree injury requiring specialist repair.

Misdiagnosed perineal tears can lead to debilitating symptoms including faecal incontinence, pain and psychological trauma.

Miss P suffered a Grade 3c tear during the delivery of her child (which was not negligently caused), however, it was incorrectly classified as a Grade 3a tear and was managed inappropriately as a result.
